Provider Selection
Families & Individuals are directed to Find a Provider by visiting the Ohio Department of Developmental Disabilities website to search for a provider.
Active Providers List - This list is used in combination with the state's list to help individuals narrow their choices to those providers with current availability.
The spreadsheet has four different tabs; one each for Independent Providers, Agencies, Day Hab Providers, and Inactive. To see the providers listed on each tab, look for the tabs in the bottom left corner of the spreadsheet.
The Inactive tab doesn't mean the provider no longer offers services, just that they have not updated their information with the County Board.
To view the profiles of each provider, click on their name. You may need to have the most recent version of Adobe Acrobat Reader to open these files. Download the free reader here.
Every effort has been made to ensure the accuracy of the information listed here. The providers themselves submit the information to us monthly and it’s then compiled into this spreadsheet. Providers who do not submit information are moved to the “Inactive” tab. For a full list of certified providers, please visit the Ohio Department of Developmental Disabilities search page.
NOTE: With the updating of the Provider Search on the State Website, Clark County DD has decided to end our Provider Availability Spreadsheet on our site January 1, 2012 .
