Community Living Services

 

Community Living Services -  Path Coordination
Community Living Services, provides “service and support” administration and acts as a single point of contact. Each individual is assigned a Path Coordinator who oversees the development and implementation of services. The SSA’s goal is to listen to the individuals being supported, facilitate services in the best interest of the individual as he/she agrees to them, write plans for those services, monitor the delivery of the services and authorize services as provided by the Board.

Intake/Eligibility Determination
Community Living Services completes all intake and eligibility determinations (ages 3+) as well as all Pre-Admission Assessment for Specialized Service Referrals (PASSR) assessments. This assessment is required for admittance to a nursing home. Residents of Intermediate Care Facilities for the Mentally Retarded (ICFMRs) are not assigned a Path Coordinator.

Community Residential Support
Community Living Services coordinates supported living, Home and Community based services (waivers) administered by the Clark County Board of MRDD and Family Support Services for those three years old and above.   Service providers are selected by the individual and monitored by Community Living Services to assure implementation and adherence to the plan.
